Co-create a personalised Care and Support Plan alongside the people we support and care for with relevant risk assessments.
Review Care and Support Plans and relevant risk assessments on a planned basis and on an ad-hoc basis, and in response to changes in individual needs.
Ensure high-quality and safe care through continuous learning and improvement and identifying and responding to concerns.
Support colleagues to understand individual care and support needs and outcomes through effective communication.
